The contact information for the order form is taken from your account info. In your editor account, click on the "Edit Your Info" link in the black navigation bar. If you change the information here, it will appear on the order form in the cookbook.
You can turn off the order form in the Layout and Design Center > Additional Design Options > Include/Exclude Order Form
